A copy of the following documents are required to complete the account opening process.*
- Valid photoID (Driver’s License, State-issued ID Card, or US Passport). The document must include first and last name as well as the date of birth.
- Social security card
- A utility bill issued to you within the last 60 days. The utility bill must be sent in its entirety including: the name of the utility company, your name, and your current residential address. Acceptable types of utility bills include: gas, electric, water/sewer, and phone/cable/internet.
Once the documents are received, the review process will take approximately 3-5 business days. The verification status can be tracked once all the documents are submitted.
A notification will be sent when the documents are successfully verified. If at any point in the verification process there is an issue, a notification will be sent. Documents can be resubmitted through the Family Money app or with one of the two alternative submission options.
*Make sure the documents are not larger than 25MB. Acceptable document formats: pdf, png, or jpeg.
Alternative Options
There are two alternative options to submit the required documents:
- Fax the documents and a cover sheet with attention to “Verizon Family Money” to 801.298.6404.
- Mail copies of the documents to:
